This release is made to allow Oxygen Cable, LLC; The Comcast Corporation and The Women'sVision Foundation ("Producers") to include me as a participant in the Mentors Walk: Bringing along the next generation ("Event"). I understand that my participation in the Event and execution of this release is deemed consideration for Producers allowing me to participate and I acknowledge that Producers will incur considerable expense in reliance on my execution of this release.

I agree that Producers may tape and photograph me and record my voice, during and in connection with the Event. I agree that Producers shall be the exclusive owner of the videotape of the Event and all copyright and other rights in and to such taping, photography and recording ("Footage") and shall have the right forever and throughout the world to use and license others to use the Footage in any manner Producers wish in any medium, whether now known or hereinafter invented.

I further agree that Producers may use and license to others to use my name, voice, likeness and any biographical facts which may have been provided to Producers including, without limitation, in the advertising and promotion of the Event, Producers or any of its productions. I confirm that, to the best of my knowledge, any statements made by me during the Event will be true and will not violate or infringe upon any third party's rights.

I agree that Producers shall have no liability for any personal injury, death or property damage I may suffer and I agree to hold Producers harmless against any personal injury, death or property damage suffered by me in connection with my participation at the Event.

I assume all risks, including any risk associated with any special medical needs or conditions of participation in the Event. I authorize medical personnel and/or organizing staff to administer any medical care treatment as the circumstances require while being transported to and from the Event and while at the place of destination. I agree not to hold such person responsible for any damages arising from the giving of such consent and agree to pay all expenses incurred due to an emergency.

I acknowledge that I am at least 18 years of age and a legal resident of the United States, and I have read the above and understand its contents.
